Perimenopause & menopause: what happens in your body when nobody explains it

Your cycle is changing, your sleep is restless, your mood tips more quickly. And the lab says everything is normal. Perimenopause is exactly the phase in which this happens.

Signs women often describe to me

  • A cycle that gets shorter, longer or irregular
  • Sleep that no longer lasts the night
  • Hot flushes or night sweats
  • A mood that tips faster than it used to

These are signs, not a diagnosis. Please always have new or severe symptoms checked by a doctor.

Why a single hormone test tells you little here

In perimenopause, hormones fluctuate widely, so a single blood test is only a snapshot. For women over 45, the UK’s NICE guideline recommends recognising perimenopause from symptoms rather than from a blood test. Values such as oestradiol or AMH are explicitly not meant for this.1

So a “normal” result does not mean nothing is changing. It means the blood test is answering the wrong question. The right question is: what are you experiencing, since when, and which of it can be supported in a targeted way?

Whether hormone therapy is an option for you is a decision you make together with your doctor. I help you go into that conversation well prepared.

I wrote a whole book about this.
