Thyroid & Hashimoto’s: when your results look fine and you still don’t feel it

You take your tablets, your TSH is within the reference range, and yet you are tired, you feel the cold and you barely recognise yourself. You are not imagining it. It is a question that deserves a more precise answer.

Signs women often describe to me

  • Exhaustion, even though you sleep enough
  • Feeling the cold, with cold hands and feet
  • Hair that is getting thinner
  • Weight that shifts although you are doing nothing differently
  • Concentration that slips

These are signs, not a diagnosis. Please always have new or severe symptoms checked by a doctor.

What your report shows, and what it doesn’t

TSH is the standard value used to adjust thyroid treatment. If it sits within the reference range, that one value fits. It does not automatically tell you whether you feel well.

Studies show this is no exception: in a Mayo Clinic study, around one in four people with treated hypothyroidism had ongoing or new symptoms despite a normal TSH, most often fatigue.1 Whether these symptoms come from the thyroid or have another cause is an open question. It is exactly the question worth asking.

Hashimoto’s is more than a lab value

In Hashimoto’s, the immune system turns against the thyroid. The tablet replaces the hormone the gland no longer makes in sufficient amounts. Findings from a smaller cross-sectional study suggest that raised TPO antibodies may be linked to persistent symptoms despite a normal TSH.2 That is why I look at the immune system, the gut and nutrients as part of the same system.

Important: your doctor decides on your medication and on any change to it. I give you reasons you can take into that conversation.

Questions about the thyroid and Hashimoto’s

Which tests make sense beyond TSH?

That depends on your situation. With your symptoms in mind, we often also discuss the free thyroid hormones and the antibodies. Which of these are tested is your doctor’s decision.

Can I replace my tablets with diet?

No, that is not the aim. Diet and lifestyle complement medical treatment. Any change to medication belongs in your doctor’s hands.

Is there an e-book to get started?

Yes, in German. In “Blutwerte verstehen” (Understanding your blood tests) I explain how to read your lab results and which questions to ask your doctor. See the e-books
